ORTHOSOCIETY CONTINUING ORTHODONTIC PRACTITIONERS STUDY CLUB DIPLOMA CERTIFICATION PROGRAMME

A. GENERAL INFORMATION AND PROVISIONS

  1. Orthosociety and Octacademy are institutions protected by the law of the Republic of Indonesia under the auspices of PT OCTACADEMY INTERNATIONAL INDONESIA and registered under the Ministry of Law and Human Rights of the Republic of Indonesia, and accredited by the Ministry of Health of the Republic of Indonesia.

  2. Orthosociety and Octacademy does not have permanent ties with any professional organization in the Republic of Indonesia.

  3. All activities conducted by Orthosociety and Octacademy will likely not be recognized and approved by the dental association in some countries. However, should in the future a partnership can be established with certain professional organizations or institutions, we will notify you verbally or in writing.

  4. Orthosociety and Octacademy are in partnership with several institutions throughout the world, i.e the City of London Dental School (United Kingdom), the International Orthodontic Practitioners Federation (IOPF), the Malaysian Orthodontic Practitioners Association (MOPA), the University of Puthisastra (Cambodia), BFC Dental (Thailand), Xpert Dental (Hongkong and Macau), and The University of Greater Manchester (United Kingdom). The number of partnering institutions may change, decrease, or increase.

  5. DIPLOMA Certification in Clinical Orthodontics is awarded by the City of London Dental School.

  6. Master of Science in Orthodontics (MSc Orth) certification is provided by The University of Greater Manchester.

  7. The Fellowship and membership status are awarded by the International Orthodontic Society.

B. ACTIVITY AND PLACE OF ACTIVITY

  1. Orthosociety and Octacademy conducts short courses and continuing Education Programmes in the world.

  2. The venue for conducting the courses will be placed in a lecture center, meeting room, or other conducive places in the capital cities in every country. Should unexpected situations occur, Orthosociety and Octacademy shall change the date or move the location of the activity to another city or even another country. All costs arising from these changes (transportation, accommodation, personal costs) are beyond the responsibility of Orthosociety and Octacademy.

C. PARTICIPATION AND PAYMENT OF ACTIVITY FEE

  1. All Participants are requested to pay the fee according to agreement.

  2. Late payment tolerance is given for 14 (fourteen) calendar days after the payment due date. If after 14 (fourteen) days the payment is still not received by Orthosociety and Octacademy, then the participant is deemed to have resigned and is subject to the provisions in accordance with article D.2.

  3. The fees for certification, level, degree, and accreditation from third-party institutions are the burden and responsibility of the participant. Orthosociety and Octacademy only acts as an accredited intermediary for these third-party institutions.

D. CHANGES AND CANCELLATION

  1. Changes and Cancellation by Orthosociety and Octatacdemy or the Third Party

  1. Orthosociety and Octacademy have the right to change the location of activities, schedule and date of activities, as well as materials and speakers should there be technical difficulties that cannot be overcome or force majeure.

  2. Changes and cancellations due to technical difficulties shall be immediately informed verbally or in writing, via email, WhatsApp, or other social media platforms.

  3. Orthosociety and Octacademy have the right to change the details of the events (e.g. speakers, venues, topics).

  4. Should the entire ongoing activity be canceled due to certain reasons (due to state administrative reasons, politics, law, natural disasters, illness or death of the speaker, as well as other force majeure causes) then Orthosociety and Octacademy shall refund the participant fees that have been deposited, which has not been used. All fees that have been deposited and have been used (has gone through a certain session) are non-refundable.

  5. Should there be a cancellation of partnership by a third party (another institution) working together with Orthosociety and Octacademy for reasons similar to point D.1. d., thus Orthosociety and Octacademy shall inform participants in writing. Activities shall continue, and matters relating to the partnership with the third party shall not change other points contained in this agreement.


  1. Changes and Cancellations by the Participant

Any changes and cancellation by the participant must be submitted in written form through email: [email protected]

Partial Cancellations by Participants:

If the participant initially chooses to take part in all the activities, then later decides to take part in only some of the activities or even withdraws, then the participant is obliged to pay the difference in costs based on the calculation of the cost per session that has been attended.

E. CONFLICT AND DISPUTE RESOLUTIONS

All conflicts and disputes shall be resolved through negotiation for consensus, and should there is no common ground reached, they shall be resolved in accordance with the applicable law in the Republic of Indonesia.


By reading, filling out, and signing this registration form as well as provisions and approval of participants, therefore I hereby declare that I understand, agree and am willing to follow what is stated in it, as well as participate in Orthosociety and Octacademy activities without coercion from any party.
